We are seeking a candidate with a strong positive attitude toward customer service to act as a Nurse Manager for our Supportive Services/Private Duty department.
This person will have a genuine interest in geriatric care and be able to motivate while maintaining relationships with staff and residents.

? The Manager, Supportive Services/Private Duty is responsible for managing the programs' admission process, assigning, scheduling, and monitoring staff to clients.
Establishes and maintains an open, effective communication with clients, family members, referral sources, physicians, and third-party payers.
Functions as the Supportive Services/Private Duty public relations person, providing pertinent information regarding the Program.
Obtains referrals from Friendship Village staff, hospital social workers, physicians, family members and other sources.
Provides information regarding services to outside resources and handles inquiry calls regarding the Supportive Services/Private Duty program and services.
Provides Home Consultations for clients within FV communities and communities at large.
Conducts admission interviews with the client and/or responsible party, obtaining all required financial data, signatures, pertinent social information, and medical information, attending physician information, and other pertinent and required documentation prior to admission.
Ensures that the resident/patient, family, and responsible person are fully informed of the residents' personal and property rights.
Reviews all admission related paperwork with the resident and or/responsible party including Medicare Part A coverage and non-coverage procedures.
Maintains all non-medical record related paperwork in an admissions file for each resident.
Assigns, schedules, and monitors SS/PD staff members.
Manages and ensures quality of care provided to clients.
Communicates concerns and/or changes in care issues to clients, families, management, and caregivers.
Maintains complete confidentiality regarding admission information and client records.
Provide advocacy, mediation, resource connection, and counseling to clients and/or family members.
Communicate information regarding new/current clients to Scheduler for scheduling and client care.
Conduct private-duty supervisory visits.
Attend Care Meetings (weekly, bi-weekly, monthly dependent on assigned community) Input, update and manage all pertinent Supportive Services/Private Duty client information in appropriate database including admissions, discharges, deaths, transfers, changes in health, assigned caregivers, payment methods, and temporary and permanent assignments.
MANAGEMENT
Responsibilities:
Performs and provides input to the Director for the selection and development of employees, employee counseling and motivation, disciplinary action, organization of goals and objectives, and planning, organizing, integrating and measuring the work performed by staff.
EDUCATION AND
Experience:
LPN degree required.
Bachelor's degree in business (or related field); or Registered Nurse (RN); or Bachelors in Social Work (LBSW) preferred.
Experience in a long-term care setting desired.
Good working knowledge of Medicare Part A and certification procedures.
Experience and knowledge of social service principles and practices relating to elderly population including intake and assessment.
Knowledge of Federal and State laws and regulations that govern home health care.
Must possess excellent written, oral and interpersonal skills, excellent follow-through, as well as the ability to juggle multiple priorities.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
